Insurance

Insurance companies do not offer benefits for psycho-educational testing.  
Dr. Claridge does not accept insurance and Dr. Claridge is not listed as a Provider with any insurance company.  Services are considered private pay.
 

Payment

Cash, check, debit, and major credit cards (Visa and Mastercard) accepted for payment. The office DOES NOT accept AMERICAN EXPRESS.

Cancellation Policy

If you fail to show for your scheduled appointment and you have not notified Dr. Claridge at least 24 hours in advance, you will be charged a non-refundable cancellation fee for cost of lost session.
